LEARNING OBJECTIVES:

  1. Student should be able to perform specialized testing including OCT, fundus photos, B scan, pachymetry.
  2. Learn to be “the doctor”. Determine what tests should be done, formulate a treatment plan and decide on follow up interval.
  3. See and treat different eye disease, and manage their time appropriately to see multiple patients.

Arrival procedure for students:

Prior to the first day of your clinical experience you will need to:


a. Schedule an appointment to have your fingerprints taken in our Human Resources Department at 608-372-3971. When you make this appointment you will need to let them know you are a student participating in a clinical experience and where you will be working. This has to be completed no later than fourteen working days prior to the first day of your clinical experience. DUE TO LIMITED TIME PLEASE SCHEDULE AT YOUR EARLIEST CONVENIENCE.


: When you come to this appointment you will need to bring two (2) valid forms of identification (driver’s license, Social Security card, passport, Military ID, etc., (we can’t accept school IDs).


b. Complete the Mandatory Training for Trainees. The instructions to do this are on one of the attached documents (TMS Mandatory Training Instructions); please make sure you follow the instructions exactly as written.

c. Complete the Application form. It is the attached document (Application VA Form10-2850). 


d. Please email the completed application to Megan.Franklin@va.gov


e. Email the most current immunization records and TB skin test results. 


f. Speak with your school to have them provide us with a Letter of Good Standing that also lets us know you have completed the necessary course work to attend this clinical experience.

The first day of your clinical experience will begin at 8:00 AM in Building 408, Room 2893. At this time, you will receive an orientation that will take approximately three (3) hours.
