Arrival procedure for students:

Need documentation of titers for rubella, mumps, rubeola, varicella, hepatitis B and documentation for Tdap less than 10 years ago. TB screening- PPD within 3 months ( documented) or needs to be placed with reading in 48-72 hrs OR documentation of current TB-quantiferon gold OR if h/o (+)PPD needs symptom check and current chest x ray. 

 

Paperwork to be completed:
1. Forms (I-9, WOC appointment letter, OF 306, 1 page application, & PIV ID form)
2. Complete the online training (2-3 modules, it varies)
3. Get fingerprinted a MINIMUM of 2 weeks prior and no more than 3 months prior to their start date (fingerprints must be redone for each rotation)
4. Signed Standards of behavior form


On first day, arrive at 7:30, go to 5th floor in middle of building in hospital, Room L5357 and ask for Dr. Marciniak.

"Overall, educational experience was the best I have received in any rotation so far. The hospital based setting allows you to see a lot of unique systemic diseases and related ocular pathology. The atmosphere can be aggressive at times, schedule is face paced, and the hours are long therefore if those things are difficult for you to handle, this may not be the best site for you. However the attendings here really push you to be a knowledgeable and independent clinician. I am so glad I chose this site, the experience and knowledge I gained here really made me a stronger clinician."
